Important Correction for Form 2-M / County Central Committee

I was just at the Summit County Board of Public Elections and the workers there informed me that on Form 2-M, when you get down the the portion where you check the second box, the Precinct is the letter after the name of the municipality. 

for instance, mine is listed as: Precinct: 4050/1 MOGADORE A 

So I enter Precinct A, City or Village of Mogadore   

Again, I apologize for the oversight.  You have until Dec. 7th at 4:00 pm to submit, but I would try to get it done By Monday the 5th of December at the latest.

Here is the form http://www.sos.state.oh.us/sos/upload/elections/forms/2-M.pdf , you will need to bring it filled out (and I recommend bringing a blank too, just in case) to:

470 Grant Street, Akron Ohio 44311

Don't be shy about asking one of the workers to look over the form for you.  They are nice and professional people in my experience.
